Waiver

In consideration of my eligibility to participate in Black Dragon Martial Arts training and/or seminars, testings, demonstrations, or other sanctioned events, I hereby waive, release, and discharge any and all claims for damages or death, personal injury or property loss/damage which might occur to me as a result of my participation in the above mentioned or any other related activities.  This release is expressly intended to discharge in advance Black Dragon Martial Arts, any and all individual Academies of Black Dragon Martial Arts, their related corporations, partners/partnerships, Instructors, employers, agents, officers, coordinators, members and representatives, as well as property owners/managers where classes and events occur from and against any and all liability claims arising out of or connected in any way with my involvement in the above mentioned activities.

I, the undersigned, fully understand that participation in Black Dragon Martial Arts will be extremely physically and mentally challenging, knowing that my training will expose me to various physical risks (including punching, kicking, grappling, falling, and other forms of hard physical contact, along with vigorous exercises).  I attest that there is no physical or mental infirmity that would hinder my involvement in the martial arts

I further understand that potentially serious accidents may occur during my involvement and it is possible to sustain lethal or serious bodily injuries and/or property loss/damage as a consequence thereof.  Informed of the inherent risks of martial arts studies, I hereby agree to assume all these risks and to release the parties mentioned in the paragraphs above who might otherwise be liable to me, my heirs or assignees for any and all damages without limitation which may occur from the above mentioned or from another student participant or member.  Finally, I realize that I am ultimately responsible financially for any and all injuries resulting from my participation and agree to carry personal health insurance independent from the above-mentioned parties.

Photography

I understand that pictures and videos will be taken from time to time in any of the classes or events and that pictures of me may be featured on the internet/social media, used in advertisements, and/or in other ways to promote and support Black Dragon Martial Arts.  I also understand I can make a formal request in writing if I do not want my pictures used.
